Kerley’s Courtyard Waiver and Release of Liability

By completing this form, you acknowledge and agree to the terms and conditions outlined below. Please read carefully before proceeding.

1. Acknowledgment of Risk:
I, the undersigned parent/guardian, understand that participation in basketball training, activities, and other physical programs at Kerley’s Courtyard involves inherent risks, including but not limited to physical injury, accidents, or other unforeseen incidents. I acknowledge these risks and assume full responsibility for my child’s participation in all activities within the facility.

2. Release of Liability:
I hereby release, waive, and discharge Kerley’s Courtyard, its owners, staff, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur while participating in activities or using the facility. This includes but is not limited to claims of negligence or fault by Kerley’s Courtyard or its staff.

3. Indemnification:
I agree to indemnify and hold harmless Kerley’s Courtyard, its owners, staff, agents, and representatives against any and all claims, damages, or costs, including legal fees, resulting from or related to my child’s participation in activities or use of the facility.

4. Medical Authorization:
In the event of an emergency, I authorize Kerley’s Courtyard staff to secure necessary medical treatment for my child. I understand that I am financially responsible for any medical expenses incurred as a result of such treatment.

5. Accuracy of Information:
I certify that all information provided about my child, including health and emergency contact details, is accurate and true to the best of my knowledge. I agree to update Kerley’s Courtyard immediately should any changes occur.

6. Photography and Media Release (Optional):
I understand that Kerley’s Courtyard may take photographs or videos during activities for promotional purposes. I grant permission for my child’s image to be used in such materials. (If you do not consent, please notify us in writing.)

By signing below, I acknowledge that I have read, understood, and agreed to the terms of this waiver and release of liability. I certify that I am the legal parent or guardian of the participant listed below and that I am authorized to execute this agreement.
